2023-24 (Current) Academic Year Submission Form
For the 2023–24 academic year, the Graduate College has been able to secure funding in order to provide a Graduate Student Health Insurance (GSHI) award of 100% of the single-student UC SHI cost, at $1,254/semester (maximum award of $2,508 per academic year). Submit your GSHI offer letter or GA offer letter here to receive your GSHI Award for the current (2023-24) academic year. Do not use this form to submit GSHI offer letters for the upcoming (2024-25) academic year; use the form below.
Once you submit your GSHI Award form, you will receive an email notification. Please keep the email notification for your records.
2024-25 (Upcoming) Academic Year Submission Form
Submit your 24-25 GSHI offer letter or 24-25 GA offer letter here to receive your GSHI Award for the 2024-25 academic year. The 23-24 GSHI Award does not automatically renew. If you have questions, email gshi@ucmail.uc.edu.
Once you submit your GSHI Award form, you will receive an email notification. Please keep the email notification for your records.
Submission Deadline
The deadline to submit the GSHI Award form is the last day of the semester for funding for that term.
- Spring 2024 funding: apply by April 25, 2024
- Fall 2024 funding (including submissions for Fall 2024 and spring 2025 funding): apply by December 14, 2024
- Spring 2025 funding: apply by May 1, 2025
